This project investigates ferroptosis, a form of regulated cell death associated with oxidative stress and mitochondrial dysfunction, as a potential disease mechanism and therapeutic target in ARSACS.

The exact molecular changes operating in cells in Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS) are not completely understood. Although several cellular alterations have been observed as a consequence of non-functional sacsin protein, how they predispose to neuronal cell death neurons remain unclear. Recent studies have identified a mechanism of cell death called ferroptosis as a possible key player in various neurodegenerative diseases. Ferroptosis is a form of non-apoptotic, regulated cell death that can be triggered by depletion of intracellular glutathione, leading to mitochondrial dysfunction and lethal lipid peroxidation. Sacsin localizes to mitochondria, and mitochondrial damage is also a prominent feature in ARSACS cells. Our preliminary data identify an enrichment for ferroptosis in transcriptomic data from cells that lack sacsin and show that these cells are highly susceptible to inducers of ferroptosis. Therefore, we hypothesize that ferroptosis plays a pathological role in ARSACS. To address our hypothesis, we will investigate the mechanisms underlying the susceptibility of ARSACS cells to ferroptosis (Aim 1), determine the effects of ferroptosis inhibitors, including a drug candidate in clinical trials, on ARSACS pathology in cells (Aim 2), and evaluate markers of ferroptosis in ARSACS brain tissues (Aim 3). Our goal is to provide new insights into the role of ferroptosis in ARSACS, thus potentially accelerating the development of therapies for ARSACS that target this pathway.

This project investigates genetic modifiers that may help explain differences in symptoms among people living with ARSACS.

A variability of symptoms exist among ARSACS patients which suggests that modifier genes might be implicated even among patients from the SLSJ region, and genetic variations rarely act alone. The team has already used genotyping and genealogical data to confirm a founder haplotype Hypothesis: modifier genes, beyond the known SACS variant, are also shared among patients. Hypothesis: modifiers may not strictly follow the same path observed for the primary ARSACS variant but may instead segregate within subsets of related individuals. They will now use regions of IBD instead of SNPs from GWAS to find causal associations with ARSACS.

This project investigates whether targeting Cav2.1 calcium channels can restore normal Purkinje cell activity and improve disease-related abnormalities in ARSACS models.

Reduced activity of Purkinje cells appears early in ARSACS and may be an important driver of cerebellar dysfunction and ataxia. Previous research showed that treating ARSACS mice with the mitochondria-targeted antioxidant MitoQ did not correct these firing abnormalities, suggesting that mitochondrial dysfunction alone does not explain the problem. Dr. Maltecca’s team has identified an abnormal accumulation of Cav2.1 calcium channels at the membrane of Purkinje cells, leading to excessive calcium entry and overactivation of KCa2.2 channels. The researchers have also identified two new small molecules that selectively inhibit Cav2.1. This project will test whether targeting Cav2.1 can restore normal Purkinje cell activity and improve disease-related abnormalities in ARSACS models. The findings could establish Cav2.1 inhibition as a targeted therapeutic strategy for ARSACS and potentially inform research on other Cav2.1 gain-of-function disorders.

The Sacs knock-out (KO) mouse is a faithful model of ARSACS, displaying ataxia, muscle weakness, cerebellar degeneration, and, as we have recently shown, learning and memory deficits. With this proposal, we seek to pharmacologically rescue motor- and cognitive phenotypes in the Sacs KO model of ARSACS with a drug that raises levels of a second messenger (cAMP) in the brain and that is currently in phase 2 and phase 3 clinical trials for two neurodevelopmental disorders. Second, we propose to develop gene therapy for ARSACS. Third, we have generated a conditional Sacs KO model, which we will use to pinpoint where and when Sacs loss-of-function causes the spectrum of ARSACS symptoms.
